Coming home from hospital is a significant transition. Having physiotherapy support in place early can help with recovery, confidence, and getting back to daily routines safely.
Blue Water Physio works with individuals and their care teams to provide mobile physiotherapy following hospital discharge, across the Gold Coast and Northern NSW.
How we support the transition home
Your physiotherapist can begin working with you at home shortly after discharge. They will assess your current function, understand your goals, and develop a rehabilitation plan suited to your home environment and daily needs.
We coordinate with hospital discharge teams, GPs, and other allied health professionals to ensure the transition is as smooth as possible, and we welcome referrals directly from hospital social workers and discharge planners.
